The accident and incident report form is used to record accidents and incidents involving volunteers, including details of what happened, witnesses, injuries and property damage. The template is designed to help Volunteer Involving Organisations record and respond to incidents that occur during volunteering activities. It supports consistent documentation of what happened, who was involved, and any resulting injuries or property damage.
